QUESTION ON "THE USE OF REPRESENTATIVE" FORM !!!

This form says:

"If you have already submitted your application, what is the name of the office and location"

My question is, do they mean when I STARTED the application (which was back in 2008?) But then this would be my husband who is the sponsor who did the application form.

Or . . .

Do they mean THIS application, the one that would be included in this "Use of Representative" form??????

Usually husband or wife, who is sponsoring the other used to fill up the application.
Use of a representative means if you are authorizing an Immigration Consultant, Lawyer or registered Agent to represent you, on your behalf, by paying their fee, with CIC for all the future correspondence and dealings with visa office. If you don't plan anything like that just disregard that form.

Hmmmm. . . just to make it clear about who completes this form . . . so I am the wife of the sponsor who completed all these forms for my father in law, who is the applicant (IMM0008, etc. )

So, doesnt my name go on this form? I thought I have to because on this form in #6 it says:

"Your representative is UNPAID and a family member (which is me)"

Would I be considered a "representative". ?

Whoever fills the form should give asked details and signature where required on the form. When I filled my wife's form I gave my name there and signed.
