Question - Legal Name Change Doc. (required)

Hi All,

I did indicate in my profile that, I had other name "Birth" .

Current Name Example: John Alston Doe

I provided my current name as : John Alston Doe
-> All Educational Certificates , forms , work has: John Alston Doe name on it.

I did indicate "Other Name - Birth" : John Alstom Doe

My Church Birth Certificate indicates: John Alstom Doe

My childhood passports till the age of 23 years old , indicate: John Alstom Doe , after that "2010" , my passport has changed to John Alston Doe , also an indication on the passport says: I had previously known as: John Alstom Doe

My current passport has: John Alston Doe [ 2010 issued / expiring in 2020]


So , on the "Upload document| section , the information says: "You indicated you have another name. You must provide information on the use of this other name (such as a letter of explanation, legal documents of a name change, etc)."

I have the "Affidavit - declaration indicating my name change , news paper indicating my name change as well" , do I really need letter of explanation .

In the upload section , I was planning to upload in a single file: Church Birth certificate + Affidavit declaration + News paper.

Do I also need to get a birth certificate from a govt. org with my current name or birth name? and upload it along with my single file upload.
