Question about Post-Graduation Work Permit (IMM5710)

I am working on my post-graduation work permit right now, and i have a question regarding to IMM5710. Under the Background Information section, question 2c), my advisor told me that I need to provide all the detail information about my entry visa, study and work permit. Is that right? If so, what is the appropriate way to write it? Anyone can give me an example?
