Question about imm 5491 #17 and 18

Hi to all,

I am seriously stressing out about some things on imm 5491.

1. #17 asks for a "notice of assessment" but I am a non resident of Canada since 2006, so I haven't filed a tax return in over a decade. Should I just include a letter explaining this? Will this sufice?

2. # 18 asks for "proof of income," but as a non resident and a business owner in Korea, how do I provide proof? I downloaded the "statement of business activity" form but it seems to apply to Canadian business owners, not non residents. Should I perhaps 1. send my 2015 income 2. record of bank deposits from my business? Are there any other documents that I should provide?

Any help and direction here would be greatly appreciated.

1. Yes, include a letter of explanation in place of the NOA.

2. Those documents should work.
