Question about extending spouse OWP

Hello,
Me and my wife came to canada on March 2015 to study English and work.
We are in MB doing an ESL program together from March to August and after that I would go to a College and she would be start working under a OWP (she only can start working after I start sudying on August 24 2015).
When we landed in Toronto 4 months ago, the immigration officer had given us a Visa only until October 7 2015 (7 months duration).
In that time I got a Study Permit and she got a Work Permit with the same duration,
We were also advised to apply for an extension 1 month before the Visa's expiration.
I decided however to apply earlier because the processing times for the Visas are very long these days.
I did all procedures in MyCIC website, but I'm not sure about what item(s) to mark in item 3 in IMM5710 form (below):
3 - I am applying for one or more of the following:
_____ A work permit with the same employer
_____ An initial work permit or a work permit with a new employer
_____ Restoration of your status as a worker
_____ Termporary Resident Permit
Also, I was researching that work visas are taking more than 90 days to be granted, our visas will expire before 90 days.
If my Visa expires before receiving the new visa, do I need to Leave Canada?
If I am allowed to stay here and wait for the Visa process to complete, would my wife need to quit her job (if she is working at that moment) until receiving the Work Permit extension?
