Question 5: Name at Birth - How to Answer?

My name on my birth certificate (and also stamped into the page opposite my biographical information on my passport) is in Irish Gaelic. I use the English version and that's whats on the bio page on my passport, my PR card etc.

Do I include this other version of the name in Q5?

Also, do you need to state your full English name too in this section, or just the alternatives?
