Hi, I cannot remember the exact dates of my employment and also my previous employment supervisor and manager have left the company.

Is it ok if i estimate the dates? and what should i fill for contact information.

They want the period covered and an honest accurate reflection - so there is no point in us telling you it's ok to estimate. If you have no other way to find out, then I guess you have no choice. If you can't work out the manager's name you could put down "Manager" along with the relevant contact details. It's about being as honest, accurate and transparent as possible. No gaps!
