Question 11: promotions and employer's address changes

Hi everyone,
I'm not sure what to put into my Question 11.
I've been with the same employer since Dec 2018. However, I was promoted twice, hence three different roles. Furthermore, my employer changed their legal address a few times (we're growing fast and had to change offices to accommodate everyone before WFH and Covid...)
So
Should I create 3 entries for each job title or one?
If one, then which title should I put in?
If three, then is it fine to put the most recent address in?

One entry should be ok, I will put my most recent title with the company, it doesn't matter how many title you had in the past in my opinion. I will also put the most up to date address of the company.
