Hello All, Please help me with this question.

Question 11, on CIT0002 (application form)

We need to mention in the chart about what we have been doing in our eligibility period. I was working during that time and what I am going to fill out in the chart column where it says,

1. where were you physically located (Employer address or My address, please help me with that)
2. city
3. country
4. name of the contact
5. Phone number
6. email address

I assume I need to provide my employer information here but where were you physically located column confuses me and seems like they are asking my address.
