Queries Regarding PR Application Filing

Hi Forum Members,

I am from India and filing my Canada PR.
I have few queries, will be really great-full if someone can give advise regarding the below:

1) In the Level of Education Field, should we include the 10th and 12th education too?
2) In the Occupation Section under Work History, they ask when did we acquire the minimum skill set for performing the activities in NOC code (do not remember the exact wording). I am post graduate in IT and the NOC description talks about computer science degree which is my bachelors. So should i mention Bachelor Graduation Date or Master graduation date?
3) When uploading Experience letters, should we include the Income Tax Returns too, as the system does not specifically ask for them
4) What all can be provided as medical exam appointment proof.
5) If Marriage Certificate enough as Proof of Wife Legal Name change document?
6) Along with Education Degrees should transcripts and ECA and IELTS TRF also should be uploaded too?

1)Where exactly?
Post-ITA Personal Activity History...? If it's within the last 10 years, then yes.
Post-ITA education history? Only if you're claiming points for just high school

2)If the degree is the only requirement to be qualified for this NOC, then yes, it's the date when you acquired degree. Highest one works good if it's related. Some other NOC like for managerial positions, require degree in related field AND several work experience years.

3)Not needed. Technically, only the reference letters are required. But you can add additional documents like tax returns, pay stubs, contract...etc. Totally up to you if you think it will support your case. Something like the first three and last three works well if you want to do that.

4)If you completed your medical exam, just the e-medical provided to you by your clinic after you complete medical exam. If you still havn't done your medical exam, it's best to wait till you have the document, not just proof of appointment. They might reject it.

5)If she changed her name after marriage, and the name of spouse is different than the one in certificate, you'll probably have to provide notarized affidavit of one-and-same person.

Thanks for the detailed replies, but for Query No 5, her name actually did not change as a result of Marriage, only the surname. Everything else on her name is the same as before.
For example, if here name Before Marriage was Sunanda Sharma, then post marriage her name is Sunanda Mehra.
Thats the only change, can you confirm if affidavit is still required for this change.

Well, change of surname (family name) is substantial. Sharma is her maiden name, and it's probably what's on her passport. Am I right?
I'm no family expert but I'm guessing you still need to show that the person on the passport is actually the person on all documents you provide.
I mean it's better to have it and not need it, than need it and not have it...

Yes you still need the name similarity affidavit.

I mean it's better to have it and not need it, than need it and not have it...
Actually on Passport, it has her post Marriage Surname, meaning my last name, and also my name is endorsed as the Spouse Name on her passport and vice-versa.
