Queries on WES documents

I have few questions:

I am processing for WES, where I have sent my educational marksheets & degree to get transcript in sealed envelop. I am sending academic records request form as well.

1. Do I only need to write For applicants only part and my college/university will write for authorized officials part?
2. Will that form be a part of sealed envelope along with transcript?
3. Anything else will go in sealed envelope?
4. Self attested degree certificate is fine? Or should it be university attested?
5. Once I get sealed envelope, can I put it in another envelope along with degree certificate and send all together to WES?

academic records form is optional...not mandatory for WES.. if you still want then here is some info:
1. Yes applicants part is must.. fill something in authorized officials part if u r sure abt that
2. Yes
3. No
4. Simply copy of degree certificate is fine.. no attestation mandatory
5. yes put smaller sealed envelope in big envelope with degree, or other documents like passport or whatever WES asked you... put copies of each semester marksheets also
