Québec form Formulaire d´engagement

In the "formulaire d´engagement" in section 7 there is the following phrase that needs to be completed:
"I reside with, and have been in a conjugal relationship with __________ since _______."

Do you have to put the date when your relation-ship started, when you moved in together or when you entered into common-law (so 12 months after you moved in together)?

There is a similar question in one of the forms for CIC (IMM1344 question #8) which specifically asks for the date when entered into common-law, so 12 months after moving in together. With this form, however, I would understand it´s the date when the relation-ship started. Am I right?

As far as I am aware, as i did some resaerch myself, the date you entered common;law is after you have resided together for one year, not when the relationship began.
