Hi! So i was reading the basic guide to sponsor your spouse (Guide 5525) and it says that i would have to provide health care not covered by public health services. Me and my spouse are doing an inland application as he's here on a student visa for now. Does that mean that i would have to get medical insurance for my spouse?

No, you don't have to buy him insurance. It just means that you and he are responsible for expenses beyond provincial coverage, like dental, physio etc.

Note that if he is in Canada without any provincial coverage or emergency insurance, especially if here long-term, one hospital stay could bankrupt you.
