Proof of Medical Examination for OWP Application

Hello, my wife is sponsoring me under the family class inland spousal sponsorship to Canada in Quebec. She is a Canadian citizen. I currently hold a PGWP that expires in January 2025, so I want to apply for an OWP. I already got my AOR, CSQ and AIP. I did my medical exam and biometrics and they have been received by IRCC. While applying for the OWP, I am asked to upload proof that I did the medical examination. When I did it, i was given a receipt with a sticker on it that has some information and my IME number, and an emedical form that says "required" at the bottom, not completed. Can i use any of those to prove that I did my medical? Or do I contact the clinic for some other document?

Also, on an unrelated note, in the application for the OWP, I need to select one of the following options:
1- Apply for a work permit with the same employer
2- Restore my status as a worker
3- Apply for a work permit for the first time or with a new employer
4- Get a new temporary resident permit (for inadmissible applicants only)

I should select the 3rd option right?
