Just 6 month statement from bank is enough?

Any letter in any particular format showing average balance of last 6 months or any other details?

Should it be attested? Should it have my photograph?

There is a format prescribed by CIC. Its a letter from bank which includes all the details, ul find it on the forum

Hi,

Thanks for the format. however, my bank has refused to give details in that format. Is it not OK to submit just the bank statements and credit card statements ? Alongwith those 2 statements, i have been provided with a bank a/c reference letter where it mentions the a/c openin date as well.
