Proof of Funds for CEC and name change proof please help!!!

I am CEC applicant applying post ITA for Express Entry i am at the page where i have to upload all my documents. Here its says provide proof of funds but i's told that i didn't required to show any. So why its asking me to upload the file. Second thing before my passport i was not used to use my surname but now i am so would a letter of explaination is enough to proof my name chnge please help

1. Write an LOE that you're a CEC applicant and don't​ need to show POF and upload it in that placeholder
2. Get an affidavit of 'one and same person'

Can you tell me the format of the affidavit i am in India at the moment from where can i get it from??

visit a public notary, they have a format available with them. Also, the format is shared in this forum, have to locate it.
