Proof of Docs for Wife Name Change - Pls Help!

Hi Friends...

I would really appreciate if someone can advice/share the list of docs I need to attache for Name Change.

My Wife full name is different (in her all educational docs + birth certificate) than what she has now in passport.

(In short - She has Old name in Educational Docs & new name in Passport & Marriage certificate)

What is the exact process & list of supporting docs I have to attached with regards to name change for my wife.

You only need to submit the marriage cert.

you wife need to provide self declaration on plain paper but it must be Notarized i already did same before submitting file as from one of my friend the demand proof of name change

Yes I have also done the same. In my case as per Indian tradition name has "Ben" suffix so I have made affidavit as one and the same person's name. Refer link below.
