Just want to know there is one form for proof of citizenship certificate form, do we need to attach this form as well when apply for citizenship

proof of citizenship certificate and apply for citizenship

These are two different things. You don't have to (well, you can't) apply for proof with your citizenship application.
I guess you may be talking about the citizenship certificate given at oath ceremony, if that is the case you don't have to attach any extra forms for that.
