Procedure for applying for provincial nomination for use with EE ?

Provincial Nominations give you an extra 600 points for Express Entry, so I'm interested in knowing the following:

1) How does one file for a provincial nomination ? I would be interested in filing with Ontario.

2) How long does the process take ?

3) When should you file for it, with respect to when you file for EE ? I intend to file for EE in early/mid July hopefully. Can I get Ontario PN by then ?

4) Does getting a PN require you to only live in that province once you go to Canada ?

well, you could start by googling "PNP Ontario," many of your questions are answered here -
http://www.ontarioimmigration.ca/en/pnp/

What you will find is:
1) Ontario does not yet have a specific EE process, but the other programs remain. There are several.
2) How to apply if you are eligible
3) How long the process takes (depends on what program you apply through)

If you want points for EE, you have to have the nomination in hand at the time you submit your profile. So apply ASAP as it does take time.

You are expected to live and work in the province that nominates you. If you happen to move after you land, however, I've not heard of anyone (yet) losing their PR.
