hello all,
My husband just had his medical done and it was found he has high blood pressure so he has to go and see a heart specialist wednesday. The doctor did give him a form IMM 1017, but I am unsure of what to do with this form and is there something else the doctor must give him before we submit the forms?
So I am just wondering if we have to wait until after the specialist appointment and what exactly is the form the doctor will give him that we submit with our application.

Form IMM 1017 should be attached to your PR application and sent together with the other supporting documents listed in the Regional Guide, it's the Medical Report - Section A mentioned in the guide, there are various versions of the form, you can read more about them here: http: // www . cic . gc . ca/english/department/partner/dmp/exam . asp. not sure if the DMP's referring you to a heart specialist has anything to do with your PR app, you should ask your doctor. Looks like it's just the doctor doing his job letting you know of the health issue that needs to be taken care of.

thanks for your reply....so IMM 1017 is the only form from the doctor I need to send with my application?
