Hi all:
So instead of posting a new topic I found this link:
http://www.canadavisa.com/canada-immigration-discussion-board/medicals-before-ita-t450343.0.html

It seems like for CEC Inland applicants Pre-ITA medical is feasible if you are fairly certain to receive an ITA in the coming draw. However, I called the assigned clinic on CIC website and the receptionist told me they can only do the medical after I receive the ITA. In that case, what does upfront medical mean? When can I go to the medical, directly after ITA or I have to wait until CIC send me the medical request after ITA?

You can book your appointment as soon as you receive an ITA.

Medical before ITA is not recommended because the results are valid for only one year and you need to land before your results expire. Many people are applying from outside Canada. It takes time for them to come to Canada and complete landing. One year might not be enough for them to complete the entire process from ITA to landing.

However since you're a CEC inland applicant, processing time is quite short for you and landing is relatively easy. It is very safe for you to get medicals done early.

Then why does the receptionist tell me I need to get ITA to do the medical? I tried the second clinic they told me the same thing (I explained to them I am a CEC applicant and I will be likely to receive the ITA soon)
