I sponsored my parents. They became PR 4 years ago. My dad passed away last December. He was the principle applicant on immigration application.

My mom needs to renew her PR card now. Will they refuse her since she was accompanying spouse? She has met residence requirement, filled taxes every year.

And when I submit renewal application, I change my mom's martial status to " Widowed", attache my dad's death certificate, copy of his PR card and passport. What else I should attach?

Thank you.

No - they won't refuse her for this reason. As long as your mother meets the residency obligation and has lived in Canada for 2 out of the last 5 years - she will be able to renew her PR card. Your father's passing is irrelevant for the PR card renewal. It's based entirely on your mother meeting RO.
