We got our PR photos from Costco and there is a stamp with the date and address of the location on the back, as needed. Then underneath it states "I certify this to be a true likeness of (applicant's first name). And then at the bottom there is a blank line for a guarantor's signature.
The photographer did not write anything for these fields. Is HE the one who is supposed to write our name and then sign? Or are we? Also, our birth date is supposed to be there but I'm not sure where and, again, if he was supposed to write this info.
Any help on this appreciated - whether I should go back to Costo and have them fill all info or just fill in the missing info ourselves.
