PR Card Renewal - been physically here last two years

Hi guys,

Here are some dates:
Landed: October 14, 2009.
Left Canada: October 22, 2010.
Returned to Canada: September 25, 2013.
PR Card expired: October 31, 2013.

I didn't renew the card since I returned because then I wouldn't have met the residency obligations. However, on October 22, 2015 I would have satisfied the 2 years residency obligation needed to renew. So, I'm planning to mail in PR renewal on October 22, 2015. However, I have a few questions in the form that are 80% clear to me but I'm just making sure.

PS: in the pictures, question #19, I mention September instead of October. Please refer to it as October (I was thinking September when I made that image). Question #31, replace "Sept 2010" with "Oct 2010" in your mind.

It clearly says "the past 5 years" for all of them so if you apply in Oct. 2015, you list since Oct. 2010.
