PR card not received - Need to enter again

Hi All,
I am a PR holder and did a short landing in Nov-15 and came back to India due to urgency here. Till now I am awaiting my PR Card to be sent to address given in Canada itself.
However now I am planning to go back to Canada but in absence of PR card I can not.
Can somebody please guide me on
1) what are the requirements for getting a travel documents from Canada embassy in India ,
2) what are the processing times ,
3) what should be justification given so that there is no denial ,
4) fees involved ,
5) is any appointment required to be taken
6) etc..
