Was your passport request really received in March 2012? Or November 2012?

When I received my passport request in September, I had the exact same request. That information is reflected on my COPR.

Confirmation of Permanent Residency.

When you send your passport, that completed eye colour/height form, and your two 35mm x 45mm photographs they use these things to generate the paperwork you need to land. Everyone gets back their passport, but in addition you will also receive back your permanent resident paperwork ("Confirmation of Permanent Residency" - two sheets of legal sized paper in my case). You may also find a "counterfoil" (one use TRV) in your passport if you are not visa exempt.

When you land, the CBSA officer will have you initial and sign both copies of the COPR. One will be stapled into your passport and the other sent to CIC, who will use it to produce your initial PR card and then keep it on file somewhere for future reference.

I received the request as an e-mail. I just printed it out and wrote down the information they requested and sent it back. No special form required.

This should be spelled out in your passport request letter - I know it was for mine (LA) and they were very clear that the only option for return was: (1) first class mail; or (2) USPS Express mail to an address in the US.

The Detroit website lists similar restrictions

http://www.canadainternational.gc.ca/detroit/visas/apply-how_a_demande-comment.aspx?lang=eng&view=d#mailing
