Could somebody tell if I need to add Schedule A (IMM5669E) along with LOE while submitting documents post ITA, as my USA B1 Visa was rejected once ?

Just declare your rejection and the details in the Statutory Questions of your eAPR. No need for IMM5669. If they need it, they will ask for it.
