Post Graduation Work Permit Extension needed

Hello I graduated on August 25 with a Master's Degree (1 year) and obtained my Post Graduation Work Permit. in December 2018. Since my passport was valid until April 2019, it says issued to validity of passport. Then I renewed my passport. Therefore, I have to extend my PGWP to get remaining months. As far as I have read in this forum, they say do similar to what I have done for the first time but I am not sure about how to fill some sections. The questions are;

1)For example it asks my current status at section 7 of imm5710e, should I write student(similar to what I did for first pgwp?).

2)It also asks the period for my current status at section 7 (I graduated on Aug 25 but study permit was valid until april and now my work permit is valid until april).

3)It also asks validity of my passport at section but I dont know if I should write renewed one or previous dates?

I saw that many people had the similar problem before, I would really appricate if someone who did it before helps.

Hi

1. Your status is worker.

2. Your status ends in April.

3. The new one.

You are not a student, your status expires in April and you have only one valid passport. There is no other way for you to answer.

My last question is should I say;

a)Apply for work permit with the same employer
b)Apply for work permit for the first time or with a new employer

For me since it is PGWP ie. open work permit, I guess it should be b) am I right?
