Post Grad Work permit extension

Hi All,

I did my Masters (2 years program) in an University here in Canada.
When i applied my Post-Grad work permit, i got just 2 years instead of 3 years.
This was due to my passport expiry date.

Now i renewed my passport and trying to extend my post graduate work permit (open work permit).
i had hard time figuring which application I should fill?

Can anyone help me please...!?!

Fill up IMM 5710; application to change conditions, extend my stay or remain in Canada as a worker.
