Police Verification for FSW in UK

Dear Friends,
I wish to apply for PR under FSW from India. I was in UK for last 1 and half years. I read somewhere that if one has stayed out of country for more than 6 months than police clearance document is also required to be obtained from that country. If this is correct, then I need to obtain the same from UK police as well. Can some one confirm or provide the link where I can apply online to get clearance from UK police. I was staying in Harrow, London. Can some one also share the fee for the same?

And just to make sure.. it's consecutively 6 months.. I stayed in UK more than 6 months but all of the stay was in 2-3 months slots in different times

I visited this link which has the required forms. But I have one query regarding the form itself. It has one column of Employer Name. I am assuming I need to mention my employer when I was working in UK and not my current employer. I moved to India last year.
