Police clearance certificate

Please correct me if I am wrong regarding getting a police clearance certificate in USA.

Step 1. Do I need to order a blank fingerprint Card from FBI. if Yes, is there any fee? How can I order it?
Step 2. submit a letter (PC-1 available in Appendix B of the Application for Permanent Residence, Buffalo) and get my finger prints on that Empty card with local police department.

Is this the process correct for getting clearance certificate? if no, please guide me..

I have a TX driver License and Can I go to OHIO local police Department to get Clearance certificate?

Any suggestions or replies are highly appreciated.

Step 1 .- Nope you can print and example from the FBI website but almost all police stations have their own cards.

Step 2 .- My experience: Go to the police dep. show your driver license and ask for State criminal background and fingerprint card to send to FBI, you will pay a fee after you got your FBI card fill the cover letter and send it to Virginia.
