Police Clearance Certificate from Spain

Hello, I need to get my criminal record certificate from Spain. However, I am a bit confused about the documents that I need to post in order to get the certificate.
1) I left Spain in 2012 and haven't gone back since, and I no longer have my old residence card with me. Would this be a problem? Should I just send a copy of my current passport? But then, how would they know that I lived in Spain?
2) The Ministerio De Justicia website states that one should substitute the original documents with photocopies officially certified or verified as a true copy of the originals. What does this mean?

If you have stayed for more than 6 months continuous in Spain, then request for PCC from nearest Spain consulate or Embassy.

But the documents need to be posted to Madrid. At least this is what the website states.
