Go to DPO office, with 4 pictures, PP copy, NIC, they will give u form, fill it and submit it. They will verify it from your respective police stations of the addresses u mentioned. Normally they provide in 4,5 days.

I have shift to another area and I have been living her for 4 years do I need to send police certificate for pervious address . I mean Do I have to send two police certificate for different area

Well u can send multiple address of same city. Ideally all addresses after age of 18 years must be covered. If u have been in more than one city then u have to take PCC from all cities.
