Police certificate from Individual states (USA)

I've lived in the state of Virginia,USA for over 6 moths in the past so I've requested the Police Certificate from the FBI for my Invitation to Apply.

I am just wondering whether there is anything I need from the state of Virginia.

You would only need an identity history summary check from the FBI.

Singe Name issue. Need immediate help

I have only "Given Name" in Passport. I have to get USA police certification as I had been there for 3 years.
In 258 fingerprint & I-783 form First Name and Last Names are mandatory. The shop who created fingerprint has filled First and Last name as
same and he told we do like that.
Last time when I was in USA in D160 I have to fill FNU as first name.
Just wondering is this correct way to do. I tried to figure out on FBI website but has not got any clue in such case what need to fill.

If some one is aware, Please let me know.

Why don;t you call FBI helpline and ask them the correct procedure. That will be the right way to ensure documentation and the PCC are issued correctly. Here is their phone number if you don't have it already - 304-625-5590 or identity@ic.fbi.gov

Thanks ! I called them but they mentioned that its your wish how you want to print. They way you mentioned the name, we will send report accordingly.

How have you or are you planning to mention your name on the EE profile? Make sure it matches your passport. Have your SSN also there - so that FBI wont reject it because of name change. Better idea - take the print of the FD 258, fill it yourself and then take it to a fingerprinting agency - some UPS stores also do it if you are based in the US.
