PNP In pand Paper Based Application - Did my medical now what?

Hi I just did my medical check at the doctor yesterday. They gave me an emedical information sheet. Am I supposed to upload this to the cic website?

I didn't see anywhere on the site that i can upload documents. from my account page, I clicked on the link that says upload documents but on that page I still can't actually upload
