Ok, I am going with the point, pls help me guys:

1. How can I write in my job status - like My job is from 2007-06 to Present (Should I write "present" or specific date, like - "2010-03")

2. My Mothers name Starts with "Syeda" It is written in my passport details, her birth certificate but this title is missing in her passport. What should I write in my form?

. What should I do with the "Date gaps" in the form if it is not required? Should I write N/A manually or leave it blank.

1. Write as 2007-06 to Present. You might not be able to write present so take out the print and write it in your handwriting with Black Pen
2. Write Syeda and where ever it is missing you can provide an affidavit from your mother.
Gaps - write NA if it is not valid in your case.

Use your judgement on filling in dates and activities etc. N/A may not be appropriate.

if I did bachelor's from 1998-2001 and then master's 2004-2006, i wud state: 2002-2003 - preparing for higher studies/entrance exams etc. Not just N/A.
