I have full custody of my daughter. I have a letter from the family court states that i have sole custody.. My daughter biological father location is unknown. My daughter reach medical submission part of her sponsorship. cic is asking for the Declaration from Non-accompanying Parent/Guardian for Minors [IMM5604] form Sign by the father. what do i do?

Submit the original letter from the family court (or a notarized copy) in English/French.
I would also suggest preparing an affidavit stating that you have tried to contact the father but he has not responded and have that legalized at the Canadian embassy. The embassy may require more info on this - i.e. the father's last known phone number, email, address, etc.
