Please help, package returned and unable to find problem.

I received my application package returned for missing supporting documents.

They highlighted applicants photos and Use of Rep form as missing.

As they also sent back both items which they highlighted in checklist, there seems to be some problem in both but I am unable to find what I missed. Please help me to guess and rectify.

1- Photos are 100% as per specifications, filled info on back of 1 photo. But I will get it done again from any other photographer.

2- In use of rep form I filled NA in section C, may be I need to leave it blank.
In signature both Applicant and sponsor signed, may be only applicant need to sign it.

3- In 0008 I filled mailing address and Residential both as home address of Principal Applicant in India,
In 1344 mailing address filled of Representative office and residential as Sponsor's Canada home address.
Now I realized it says fill mailing address of Representative if using any so this way I should filled mailing address in 0008 also as Representative Canada. Am I right or wrong?

4- In 5562 section b do I need to fill travel details of spouse (sponsor) also if he is already Canadian citizen?

Any help is appreciated

No need for any covering letter, just send it all back with the corrections made or in your case with the missing supporting documentation.

What kind of application? What were the documents you submitted?

So your rep is your sponsor in Canada, correct? Then it sounds like you did it right as far as I can tell.

Sorry, I just edited to clarify

Sponsor is not the Representative, its paid Representative


In 0008 I filled mailing address and Residential both same as home address of Principal Applicant in India,
In 1344 mailing address filled as Representative office and residential as Sponsor's Canada home address.

Now I realized it says fill mailing address of Representative in form so this way I should filled mailing address in 0008 also as Representative Canada. Am I right or wrong?

If you had a different mailing address from applicant's residential address, you need to include the use of representative form. It doesn't matter who the address is for - a paid consultant, the sponsor, their parents, anyone - CIC will only communicate with the applicant (the person who wants to move to Canada), unless the applicant has included the use of representative form giving them permission to communicate through someone else.

I don't want to use different mailing address but its mentioned in form "If using representative, use his address in mailing address". I don't know why they asked to send Rep form again, I think may be I filled my own mailing address and they want mailing address same as Rep address

Hi Simmi100, did you get any answer on your question #4? I'm in a similar situation and understand that the Canadian citizen sponsor/spouse does not need to fill out a separate form. However, what I do not understand is whether the Principal Applicant who is being sponsored by their Canadian spouse needs to fill out the Travel History of the Canadian spouse in the form that the Principal Applicant is filling, i.e., 2 (b) where it requires their spouse's (Canadian or not) travel history. In my case, due to the extensive travel, it will really delay the submission is that needs to be provided in the Principal Applican't Form IMM 5562. Any feedback that you can provide would be really helpful.
