I submitted my wife application on 6th Sep 12. I just rececived approval letter an email. Since me and my wife haven't got anything. Status is like same no changes. I am geeting this meesage @We received ******* application for permanent residence on September 6, 2012."

Medical results have been received.
She is in pakistan.

That's normal. Pakistan has the longest processing times in the world for CIC and it seems they often don't even look at applications for 9-12 months after they arrive in that office. Indeed, to help with the backlog, some of the files for Islamabad are now being processed in Ottawa.

You should start ordering GCMS notes (via the "access to information and privacy" mechanism on the CIC website) periodically. That will tell you what has (or has not) been done with her application. You must have her written permission to do so and there is a $5 fee for you to order her case notes.

If you are a PR, you must remain resident in Canada throughout the process. If you are a Citizen, you are free to spend the process with your wife in Pakistan, but must be able to show your plan on settling back in Canada once she is granted PR.
