Hello everyone thank you for reading, first I'm a new on this site I've learned from here for a while. And to day I just sent my files under CEC to them but the problem is I let my brother did it for me ( with FedEx)I gave him the full address like this:

After I got home from work he show me that paper. He wrote on

That is? !!! He told me because of the space on that form not enough another thing is envelop bigger then they request. So is it gonna be problem with that I really don't know about it. Please help thank you so much.
