Pleas Help!PleaseHelp-How do I fill an Open study permit for my 4 year old child

I am confused here with filling an open study permit application. I applied for a visit visa for my 4 year old child to accompany me as a dependant, in the purpose of visit i had written"accompanying my mother" and in the invitation letter i had put my visa and letter of acceptance but her visa has been denied on "Purpose of visit and proof of ties to Canada and home country". I am applying for an open study permit for her now but I am stuck in the following questions as I don't know what to fill in there since she does not require a letter of acceptance
Please help what do i fill in the following

1. Name of school
2. My level of study would be
3. My field of study would be
4. Complete address of school in Canada

I really do not want to go wrong this time when filling the form.

It sounds like you are coming to Canada as an international student. If that's the case, you will probably get the best advice by posting to the International Students section of the forum. Good luck.
