I had a spine surgery in 2018 that ended up by becoming a wheel chair user for a year before starting to walk again. I am currently unable to do any extreme physical activities but can walk using a crutch. I already received my ITA should I mention everything about my injury ? Also what are my chances in passing the medicals?

This should come up in your medical. Your reliance on crutches and whatever lead up to you needing spine surgery should be something that is mentioned in your medical.
