There are lots of different ways, there is no perfect way :)

I included all that you have listed, the exact date and I also added the names and relationships of anyone in the pictures.

I set the images out in a large document (six images to a page, on plain paper).
I added the details and description below each image, then printed the whole lot.

What did I use? Adobe Lightroom on a Mac.

There are lots of perfectly acceptable ways to label your photos.
Include the names of the people in the photo, the date (at least approximately), the place, and why this photo is included. Some people include who took the photo; I only did if it was the applicant or sponsor.
Some people use their own names, some just say 'sponsor and applicant'. I was very specific for the first photo - saying "this is a photo of the sponsor, Alice Doe, and the applicant, Peter Jones, ... . But after that I just said 'my husband' and 'I' or 'me'.
If you are printing the photos on A4 paper using Word, the captions can be a bit more detailed.

For example:
This is a photo of the first time I met my husband's parents, John and Jane Smith. We are in their home. August 21, 2006.

This is a photo taken right after our civil wedding ceremony at Enugu City Hall on September 23, 2007. From left to right: Johnson Osakwe, an old family friend of my husband's parents; Jerry Smith, my husband's oldest brother; my husband; me; Dwight Smith, my husband's cousin; and Kevin Smith, my husband's brother.

Thanks for the replies.

What i did was very simple (i wish i could upload a PDF file here so everyone could take a look at it).

I inserted the images into a Word document, 2 per page, and wrote a small description right below it.

This is the info i included.
1)Names (Left to right)
2)Date
3)Location
4)Activity

I hope this helps anyone who has any concerns on how to lable their pictures.

Good luck to you all....
