Permanent Address for Visitor Status to Canada on OUtland Application

Thank you to anyone who can help me or point me to the information. I have been unable to find it.

For 6 years I have been living between the US and Canada. I am a US citizen and have a visitor status in Canada. My residence in the US is a family member's home. I own a house in Canada with my Canadian-citizen husband.

I am applying for my PR in Canada under the outland process.

As I fill out the form, I am residing in Canada. SO the questions are as follows:
1) Form IMM0008:
Question #10. Do I put Canada as my current place of residence and do I use the dates as beginning six years ago? There is no place to point out that I was also living in the states during that time. Not sure if this matters?

Contact Info: Which address should I use for my Residential address? Is it a problem if I use the Canadian address?

2) Form 5669E:
Question 8 Personal History & Question 12. Addresses:
Do I list both addresses (Canadian and US) with overlapping dates?

3) Form 5406E: do I use the Canadian or US address as my present address?

3) Form 5490E - Is this necessary for my husband (Canadian citizen) to fill out if I am showing on the forms that I live in Canada with my husband?
