PCC Stamp on Passport

I had gone to my local passport office today to get a PCC.
They issued me 2 things

1. A document that states my PCC and the fact that it expires as soon as my passport expires, which is 3 months from now.
2. A stamp on my passport saying that I have PCC issued for the country of Canada.

which of these do I need to upload with my application?

My problem is a little convoluted as I am about to apply for a fresh passport soon , So will I need to get a PCC stamp on the new passport as well?
Has anyone faced something similar and can help?

I don't know about the PR but when my wife and I applied for her Student Visa and my dependent visa, we only uploaded the PCC certificate. I had 3 other PCC's I needed to upload along with my Indian one so I scanned all of them, clubbed them together as a single pdf and uploaded that on the CIC website.
