Pathway for express entry?

I am working as a caregiver or health care assistant in an aged care resthome and hospital - planning to apply with the same job in Canada. I already have 4 years of experience. However, I did not work from August 2018 until August 2019. Based from NOC or what I understood, I need to have 2 years of experience within 5 years. Also, I would like to know if NOC 4412 is the correct skill level for me since I mostly take care of elders or what is the best pathway? Thank you

NOC 4412 is not eligible for Express Entry but that is the correct NOC for caregiver. You can look at the Home Support Worker Pilot if you have a job offer.
