Passport Request

Hello All

Can somone please let me know what all is to be sent to CHC-Buffalo once a PPR is received?

Do they send some form which needs to be filled in,photos to be sent alongwith the passport?Please help

How did you get your passport request? Because when I got mine, in one of there were an instruction, 2 appendixes, telling the specification of photos (2 photos), and a form for proof of funds, and also you have to attach the mentioned letter to all these, without that letter they don't accept.
