Parent Sponsorship Income with Maternity EI

Hello there,
I hope someone can help me out...
I am currently on mat leave and receiving EI and child care benefits. I have some questions when I fill out the financial evaluation form:

1) Am I still counted as employed? I think the answer is yes, but I would like to confirm. I am on leave but I am still the employee of that company.
2) I know EI is income. Shall I include it in my current job income?
