I am assuming you are sponsoring your spouse from outside Canada, because you didn't really specify.
I will try to help you with your questions-

1) #10, just say YES to Canadian Citizen, and put your date of birth in the next spot when they ask the date you became a citizen. Leave all of #11 blank, or put N/A for "not applicable"

2) If you have absolutely no income, this might be a concern, because you need to prove that you can support your spouses' basic needs. But if you do have some income from any type of benefits, baby bonus etc, it might be enough. If you have "interest income", you can include proof of this as well. As long as you are not on social assistance, it should be fine. The other thing is that you are residing in Italy, so you need to provide proof that you are settling in Canada upon the completion of the sponsorship (see the Guide, and IMM 5540, Question 11), so if you have employment arranged for your husband in Canada, you can write a letter of explanation with proof.

3) NO, the sponsor needs no medical or criminal checks.

4) YES, you need to prove your relationship. The officer processing the case has seen/heard anything and everything... it doesn't matter that it's personal. You should include as much proof as possible, and if you don't want to send personal letters, send other things like joint bank accounts, leases, statutory declarations from family members and friend, proof of trips you've gone on etc. If your application is refused because they don't think you have a genuine relationship, your looking at a long, expensive and frustrating appeal process. Might as well give as much evidence as possible. If you are interviewed... they will most likely ask MUCH MORE personal questions, than the contents of your letters.

5) It doesn't matter if you are Canadian. You need to prove you can meet your spouse's basic needs. But if you are sponsoring ONLY your spouse (no kids involved), it most likely won't be a big deal. Just provide them with all information asked for, anyway.

I hope this helps.

Just a little FYI, Spousal Sponsorship applications are exempt from the minimum income requirement. In other words, you don't have to prove your income. This only applies when sponsoring someone other than your spouse.
