So im going to apply outland for PR and get sponsored by my Canadian gf she is moving back in a couple of weeks.

The plan is for me to visit for 6 months while application is processing and then come back and go for another six.

She will have a house and a job so she will provide for me and ill have a small amount of savings.

So for the outland application doe we both need to apply or just me apply and I state she is my sponsor?

What do I do about the medical and police check shall I do that before I leave here in the UK or can I do it over there?

Have you and your girlfriend lived together continuously for at least 365 days (NOT 364, but a full year)?

If so, and you have the supporting `proof', you can apply right now via an Outland application. If not...you will need to wait until you have 1 full year of cohabitation before you can apply.

When you are ready to apply, you will send everything together (Her paperwork + your paperwork + the evidence that you are providing of your relationship as well as proof that you meet the common-law eligibility) to the same address in Mississauga, ON.

Medical and police check should be included with your application(s).

Upon entry to Canada, officers might issue you a visit record if you don't tell them you will be leaving Canada within 6 months or less.
You can extend ur visit within Canada one month before your visa is expired.
the medical check you can get in Canada with a recognized physician .
the police check I'm not sure because I'm not from the UK.

We shall marry when im over there! We have known each other for four years of which we have been together for 2yrs with two breaks in between one for 1yr and recently until now a 6 month break.

Does that change anything?

No, that doesn't change anything.
I believe getting married is great but I believe you can already apply as common-law relation because you've lived together for over a year.
As you're from a visa exempt country, I'm not sure what you it is like for you to cross the border.
I'm not sure if you should tell them you're getting married or not.

visa exempt means you do not require a visa to visit canada. it depends on which country you are from.

if you are not married, and don't have a PR app submitted, yes the immigration officers at the border can give you a difficult time crossing if you can not adequately prove your ties to your home country (ie: job and housing).

If you had a 6 month break during that year of living together, you are not common-law. If you want to apply for sponsorship, you will need to either restart the common-law clock and live together for a full year or get married.
